The Key Strategic Partnership for Health and Well-being is Health Challenge Swansea.

The Partnership Vision is:
By 2020 Swansea will be a place where:
  • Everyone is supported to achieve the best level of health and well-being possible, living longer healthier lives;
  • Appropriate care is delivered in the best location at the right time by a range of organisations working together;
  • Communities and individuals are well-informed and take responsibility for their own health and well-being.

Health Challenge Swansea

Health Challenge Swansea and the Partnership Board were set up to identify and achieve actions outlined in the Health, Social Care and Well-being Strategy to improve health and well-being in Swansea. 

Membership of Health Challenge Swansea Partnership Board comprises members of of the Local Health Board, Swansea NHS Trust, Social Services, City & County of Swansea, Community Health Council and the voluntary sector. Details of the Health, Social Care and Well-being Strategy 2005-2008 and Health Challenge Swansea’s Action Plan can be found on the Health Challenge Swansea website.

August 2008 Update:
 
Health, Social Care and Well Being Strategy 2008 -2011 - Now available

From April 2003 the Local Authority and Local Health Boards have a statutory duty to prepare and implement Health, Social Care & Well Being strategies. They have a duty to co-operate with bodies such as the Voluntary Services.
The second Health, Social Care and Well-Being Strategy for Swansea is now avaliable, it aims to improve the health, social care and well-being of the population over the period April 2008 - March 2011. Developed by the Health Challenge Swansea partnership it builds upon the first strategy, which covered the period 2005-2008. The members of the Health Challenge Swansea partnership include: The City and County of Swansea, Swansea Local Health Board, Swansea NHS Trust, National Public Health Service, Swansea Community Health Council and Swansea Council for Voluntary Service.

Local Health Board

The key role of the Local Health Board is to implement strategies which will improve the health of the population.  The Board is constituted to ensure there is a balanced representation from all of the key stakeholders.  
 
The membership of the Local Health Board comprises representatives of the Local Authority and Health Authority, pharmacists, dentists, optomotrists, nurses, the voluntary / third sector, the public and local GP’s.

Planning Groups

Joint Planning Groups in specific service delivery areas enable representation of key stakeholders in the planning process.   Planning Groups in Swansea's Health and Social Care sector include:
